Our Office Policies

Our office is conveniently located on 15600 NW 67th Ave in Miami Lakes, FL. Our friendly and efficient Front Office Staff will assist you to arrange an appointment that is most convenient. If you have any questions please call us at 305-306-0600.

Office Hours

You may schedule an appointment with a patient care coordinator.

Appointments/Cancellations

With the exception of serious emergencies, it is expected that you keep all of your appointments. If you need to reschedule an appointment we require 24 hours notice. In such a case, please call our office and arrange for a make-up appointment with one of our patient care coordinators.

Emergencies

Our office provides on-call service for emergencies. Call 305-306-0600 so that the physician on call can be contacted.

For your convenience, we also accept cash or check as admissible forms of payment.
